PARENT PERMISSION FORM FOR FIELD TRIP PARTICIPATION

Dear Parent or Legal Guardian:

Your son/daughter is eligible to participate in a parish/school-sponsored activity requiring transportation to a location away from the parish building.  This activity will take place under the guidance and supervision of employees from Saint Robert Parish/School.  A brief description of the activity follows:

Name of Event:  Cedar Point Trip – We have limited space on the bus - first come first served

Destination: Sandusky, Ohio

Designated Supervisor of Activity: Josh DePalma / Maureen Kelsey                    

Date and Time of Departure: Wed., June 25, 2008 at 6:30 a.m. from St. John Vianney

Date and Anticipated Time of Return: Wednesday, June 25, 2008 at midnight.

Method of Transportation: Blue Lakes charter

If you would like your son/daughter to participate in this event, please complete, sign and return the following statement of consent and acknowledgment.  As parent or legal guardian, you remain fully responsible for any legal responsibility that may result from any personal actions taken by the named student.
